After Your Stay
Your physician will generally let you know before you will be released from the hospital. Before your dismissal, it's a good idea to make a list of any questions you may have about your follow-up care.
When your physician has approved your release, your nurse will provide written instructions about your dismissal and follow-up care.
Fidelity Health Care offers a comprehensive range of home healthcare services throughout the Miami Valley region, because they understand that recovery does not end with your discharge from the hospital. Fidelity's clinicians are experienced professionals trained to meet the specific needs of each individual patient in the comfort of his or her own home. Fidelity's services include:
- Registered nurses, licensed practical nurses, home care aides, and rehabilitation therapists
- Medical social workers
- Infusion therapy
- Private duty care
- Oxygen therapy
- Home care medical equipment
Fidelity also offers disease management programs for congestive heart failure, respiratory disease, and other specialty areas.

The Integrated Care Management staff is available to help you plan for any special care you may require after you leave the hospital. Case Managers can assist you in making the necessary arrangements for additional nursing care, therapy or special medical equipment for your home. Referrals for counseling and support for personal, family, or financial problems are also available. If you would like to see a member of the Integrated Care Management staff ask your nurse to contact the department.
