Please call to schedule a surgery. Surgery time and PAT time will be given by the scheduler.

Complete the UVMC Surgery Order Sheet
Order Sheets are available for download.
- All fields need to be completed
- These orders will be for PAT and Day of Surgery
- Without this set we will not be able to process your patients
- Surgeon must sign order sheet (Order sheet serves as PAT/pre-op orders; therefore, no further action is needed by the surgeon)
- It is necessary for you to complete the order sheet in your office and fax it to:
- Central Scheduling at (937) 335-5064 for surgeries being scheduled at UVMC, or
- Hyatt Surgery Center at (937) 667-5660 for surgeries being scheduled at the Hyatt Surgery Center
