Credentialing

Dear Applicant:

Thank you for your interest in membership and/or privileges at the Premier Health hospital facilities. Premier Health (Premier) developed a system initial application to eliminate the need for multiple facility credentials application processing. If you are applying at more than one Premier hospital facility at this time, many of the items associated with the primary source verification process will not be duplicated. If you are an Allied Health Professional (physician assistant, advanced practice nurse, perfusionist, etc.), please download the system Allied Health Professional application.

Please take time to thoroughly review the application, application fee structure, medical staff category requirements, and supporting documentation requirements. It is the responsibility of the applicant to provide a complete application to ensure timely processing. Incomplete applications cannot be processed and will be returned. Please note that completion of the application does not guarantee acceptance for membership or clinical privileges by any of the Premier Health facilities.

Upon completion of the primary source verification process, your application, with all related verification and supporting documentation, will be sent to each applicable Premier hospital facility. The individual facility will then complete the privileging and decision making portion of the application process and notify you of the Board's decision.

Please note that you are not approved to practice at the applying Premier hospital facility until you have received written confirmation from the Medical Staff Office confirming your approval to practice. It will be your responsibility to confirm this with the Medical Staff Office prior to providing any patient care.

Temporary privileges are not generally granted and are limited only to those given to fulfill an important patient care need. An applicant desiring temporary privileges shall be required to delineate, in writing, the specific patient care need which justifies the granting of the temporary privilege.
