Payment Options

Your payment’s due date will be listed on your statement. You may pay your account using cash, check, Visa, MasterCard, or Discover. Payment can be made in the following ways:

• By mail: Please detach the bottom portion of your billing statement and include it with your payment in the envelope provided.
• By phone: We can accept a credit card payment over the telephone at (937) 208-2777 or (800) 544-0630 from 8:00 a.m. to 4:30 p.m. Monday-Friday. Be sure to have your account number available when you call. Your account number is listed at the top of your statement.
• In person: Payment is accepted at UVMC’s cashier's office located in the Dettmer Building on the UVMC campus. Office hours are 8:00 a.m. to 4:30 p.m. Office staff can be reached at (937) 208-2777.

Financial Assistance

Upper Valley Medical Center’s mission is to care for all persons in Miami County who are in need of medical care, regardless of their ability to pay. To this end, UVMC offers a variety of financial assistance programs for qualifying patients.

Government Programs

The Advocacy Program ensures that all individuals in Miami County are able to receive necessary medical care regardless of their ability to pay. The State of Ohio offers assistance for patients who are residents of Ohio, not currently enrolled in any Medicaid program, and who meet the Federal Poverty Guidelines. An application must be completed for this program.

UVMC sponsors a program that provides relief from hospital bills if the patient’s family income is below 300% of the Federal Poverty Guidelines. Discounts range from 15% to 50%, depending on family income and number of family members. For this financial assistance, patients must provide proof of income and complete an application.

UVMC Payment Plan Options

Upper Valley Medical Center (UVMC) offers payment plans for 12 to 18 months for any balance that a patient cannot pay in full at one time.
