Patient Scheduling and Registration
Outpatient Scheduling
The Upper Valley Medical Center (UVMC) Central Scheduling schedules exams for outpatients. The scheduling phone number is (937) 440-7111. Hours of operation are 8:00 am to 6:00 pm Monday through Friday and 8:00 am to 12:00 pm on Saturday.
A completed physician order is necessary for these exams to be scheduled. The order must include the patient’s name, exam to be performed, diagnosis/signs/symptoms, signature of the ordering physician, and date and time of order. The order may be scheduled by phone by either the patient or the physician’s office. The order can then be faxed to Central Scheduling at (937) 335-5064, or the patient may bring the order with him/her the day the test is scheduled. View our various locations and hours for testing.
Registration
Patients who come to UVMC for outpatient testing such as x-ray, EKG, laboratory and other routine tests should report to the Outpatient Services Registration desk in the main lobby. Hours are 6:00 am to 9:30 pm Monday-Friday and 7:00 am to 3:30 pm Saturday/Sunday.
Patients who come to UVMC for surgery or preadmission testing (PAT) should report to the Surgery Welcome Center on the second floor. Registrar hours are 5:30 am to 1:30 pm. After 9:30 pm on weekdays or 3:30 pm on weekends, patients are asked to enter through the Emergency Department and check in at the ED front desk.
Patients are registered in order of arrival. The registrar will collect or update patient demographic information, have the patient sign required forms, scan insurance cards and other documents, and collect co-pays. Hospital escorts will help any patient requesting a wheelchair or assistance.
Registration Documents to Bring to the Hospital
Outpatients or patients checking in are advised to bring the following documents with them to make the registration process faster and easier:
- Physician’s order for test/admission if your physician provided one
- Insurance card
- ID/Driver’s License
Parking
For the convenience of patients, their families, and visitors, Medical Center employees are asked to park only in specially designated areas. Parking designations include:

Hospital Directory
We maintain a hospital directory listing the name, room and extension, general condition and, if you wish, your religious affiliation. Unless you choose to have your information excluded from this directory, the information excluding your religious affiliation will be disclosed to anyone who requests it by asking for you by name. This information, including your religious affiliation, may also be provided to members of the clergy. You have the right during registration to have your information excluded from this directory and to restrict what information is provided and/or to whom.
If you choose to be excluded from the hospital directory, you will receive no calls, mail, or flower delivery. The hospital will tell callers—including family members, friends, and clergy that you have not given your direct hospital phone line to—“I’m sorry, this person is not in our hospital directory.”
For reasons of privacy and confidentiality, no information will be revealed to anyone about a patient who chooses not to be included in the hospital directory.
