Mark Zunkiewicz, MD
Dr. Zunkiewicz joined the Upper Valley Orthopaedics team in the summer of 2010. He received his medical degree from the Medical College of Ohio at Toledo. He served a residency in orthopedic surgery at Allegheny General Hospital in Pittsburgh and a fellowship in orthopedic sports medicine and arthroscopy at Mississippi Sports Medicine and Orthopaedic Center in Jackson, Miss. Jeff Rayborn, MD
Dr. Rayborn holds an undergraduate degree from The Ohio State University and attended medical school at the University of Cincinnati. His three-year residency in family practice was at Tri-Health Bethesda North Hospital, followed by a one-year fellowship in sports medicine, also at Bethesda North. He has returned to the area a dozen years after graduating from Ansonia High School. Dr. Rayborn works with area athletes, serving as primary care sports medicine physician at the Center for Sports Medicine and providing game coverage. He focuses on non-operative orthopedics such as shoulders, elbows, knees, hips, fracture care and injections. Read Dr. Rayborn's full bio.
Physical Therapists and Physical Therapist Assistants
Jennifer L. Jones, PT, Supervisor/Physical Therapist, UVMC Center for Sports Medicine
Jennifer Jones has served as a supervisor/physical therapist at UVMC Center for Sports Medicine since 2001. In this role, she supervises the sports medicine staff members and focuses on the marketing, growth, and development of the department. Prior to this role, she served as a physical therapist at UVMC Center for Sports Medicine from 1999 to 2001.
Jennifer received her Bachelor of Science in Physical Therapy degree from Bowling Green University in consortium with Medical College of Ohio in Toledo, OH in 1998. She has her Ohio Physical Therapy License and is certified in First Aid and CPR/AED.
Brian E. Jans, PT, DPT, SCS, Physical Therapist, UVMC Center for Sports Medicine
Brian Jans has served as a physical therapist at UVMC Center for Sports Medicine since 2003. In addition, he also served as the Clinical Education Coordinator at Upper Valley Medical Center from 2001-June 2012. He has been practicing physical therapy since 1994.
Dr. Jans received his Masters of Physical Therapy from The College of Osteopathic Medicine of The Pacific in 1994 and his Doctorate in Physical Therapy from Western University of Health Science in Pomona, CA in 2006.
Dr. Jans is a Board Certified Sports Medicine Specialist through the American Physical Therapy Association and a credentialed clinical instructor certified by the American Physical Therapy Association. He is an avid cyclist and a certified bicycle fitter through the Serotta International Cycling Institute. Dr. Jans has his Ohio Physical Therapy License and is certified in CPR/AED.
Mindy Penn, PT, DPT, Physical Therapist, UVMC Center for Sports Medicine
Mindy joined the Physical Therapy staff at the UVMC Center for Sports Medicine in November 2012. She worked in St. Louis, MO, prior to this time specializing in Aquatic Therapy, Women's Health Therapy, and the use of Shirley Sahrmann's Movement Systems Impairment.
Mindy received her Doctorate in Physical Therapy from Washington University in St. Louis in 2005. She has completed Level 1 of the American Physical Therapy's Association Certification of Achievement in Pelvic P.T.
Mindy is also licensed by the Ohio Physical Therapy board and is certified in CPR/AED.
Greg Schultz, PT, DPT, CSCS, Physical Therapist and Strength & Conditioning Coach, UVMC Center for Sports Medicine
Greg Schultz has served as a physical therapist at UVMC Center for Sports Medicine since 2004. He serves as the strength & conditioning coach at Tippecanoe High School and instructs the “Explosive Speed and Power” sports enhancement program for high school and college athletes in the area.
Dr. Schultz received his Bachelor of Science degree in Health and Sports Studies from Miami University in Oxford, OH in 1998. He received his Masters degree in Physical Therapy from Shenandoah University in Winchester, VA in 2002, and received his certification as a Strength and Conditioning Specialist Certification in May 2004. Dr. Schultz also received his Doctoral Physical Therapy degree from Shenandoah University in 2005.
Prior to joining Upper Valley Medical Center, Dr. Schultz worked as a physical therapist in Frederick, MD. He is Ohio board licensed as a physical therapist and certified in CPR/AED.
Angie Strong, PT, DPT, MTC; Physical Therapist, UVMC Center for Sports Medicine
Angie Strong joined the Physical Therapy staff at the Center for Sports Medicine in May 2010. She started her career as a physical therapist at UVMC in 1999, previously at Outpatient Care Center North before her addition to the sports medicine staff. Angie currently serves at the Clinical Education Coordinator for Upper Valley Medical Center.
Angie received her Bachelor of Science in Physical Therapy degree from the University of Toledo in consortium with the Medical College of Ohio in 1999. She received Manual Therapy Certification in 2005 and her Doctor of Physical Therapy degree from the University of St. Augustine in 2007.
Angie's experience as a Division I collegiate volleyball player at the University of Toledo has given her a unique understanding of sports related injuries. She also specializes in women's health, therapy for pregnancy related musculoskeletal conditions and utilizes biofeeback to treat incontinence. Angie has her Ohio Physical Therapy License and is certified in CPR/AED.
Jodie Gibson, PTA, LMT; Physical Therapy Assistant, Licensed Massage Therapist, UVMC Center for Sports Medicine
Jodie joined the physical therapy staff at the Center for Sports Medicine in December 2009. Jodie also has gained experience while working as pool staff for Home Health and inpatient rehab at UVMC.
Jodie received her Physical Therapy Assistant degree from Rhoades State College in July 2009 and her Massage Therapy certification through the Miami Valley Career Technology Center in July 2000. Jodie has her Ohio Physical Therapy Assistant and Massage Therapy licenses' and is certified in CPR/AED.
Athletic Trainers
Corinne L. Lyons, LAT, ATC, CSCS, Lead Athletic Trainer, UVMC Center for Sports Medicine
Corinne Lyons has been a part of the Upper Valley Medical Center team since 2003, and has served as the Lead Athletic Trainer for the UVMC Center for Sports Medicine program since 2007. She serves as the certified athletic trainer for Russia High School, as well as an instructor for the “Explosive Speed and Power” Sports Enhancement Program and the Sportsmetrics™ Knee Injury Reduction Program.
Corinne received her Masters of Art in Physical Education degree from Western Michigan University in Kalamazoo, MI in 2003. She also received her Bachelor of Science in Sports Medicine with Distinction degree from Ohio Northern University in Ada, OH in 2001.
Corinne is board certified as an athletic trainer by the National Athletic Trainers Association, as well as by the State of Ohio licensure board. She is a Certified Strength and Conditioning Specialist, a Certified Sportsmetrics Instructor, a certified CPR/AED Instructor, and an OHSAA-certified wrestling general weight assessor. In addition, Ms. Lyons is also a member of the Greater Dayton Athletic Trainer’s Association.
Annette J. Bair, LAT, ATC, Athletic Trainer, UVMC Center for Sports Medicine
Annette Bair has served as the athletic trainer for Miami East High School as part of the UVMC Center for Sports Medicine team since 1997. She also assists in the clinic setting, instructs CPR and Sportsmetrics™ Knee Injury Reduction classes, as well as organizes multiple Pupil Activity Validation (PAV) courses each year.
Annette received her Master of Science in Physical Education from Utah State University in Logan, UT in 1997. She received her Bachelor of Science in Physical Education with an emphasis in Athletic Training from Washington State University in Pullman, WA in 1994.
Annette has her National Athletic Trainer’s Association Certification and Ohio Athletic Training License and is certified in CPR/AED. She is a certified Sportsmetrics™ instructor, CPR instructor, and an OHSAA-certified wrestling general weight assessor.
Stephanie Burdette, LAT, ATC, Athletic Trainer, UVMC Center for Sports Medicine
Stephanie Burdette has served as an athletic trainer at UVMC Center for Sports Medicine since 2008, and has worked as a certified athletic trainer in various college settings since 2003. In her current role, she provides athletic training services to Piqua High School and Edison Community College, and assists in the clinic setting at UVMC’s Outpatient Care Center North.
Stephanie received her Bachelor of Arts in Athletic Training degree from Baldwin-Wallace College in Berea, OH in 2003. She also received her Master of Education degree from Northwestern Oklahoma State University in Alva, OK in 2005.
Stephanie is a National Athletic Trainers Association member and is certified by the National Athletic Trainers Board of Certification. She is also a licensed Ohio Athletic Trainer, certified CPR instructor, and an OHSAA-certified wrestling general weight assessor. She is also a certified Sportsmetrics instructor.
Ryan Ingley, LAT, ATC, Athletic Trainer, UVMC Center for Sports Medicine
Ryan joined the UVMC Center for Sports Medicine as an athletic trainer in the summer of 2009. In his current role with UVMC, he provides athletic training services to Milton-Union High School.
Ryan received his Bachelor of Science in Exercise Science with a concentration in Athletic Training from the University of Toledo in Toledo, OH in 2007.
Ryan is Ohio OT/PT/AT Board Certified and National Athletic Trainer’s Association Certified as well as being a CPR Certified Instructor and OHSAA Wrestling Weight General Assessor. Ryan is also a member of the National Athletic Trainers Association.
Amanda Ingold, ATC, LAT, CES; Athletic Trainer, UVMC Center for Sports Medicine
Amanda joined the staff at the UVMC Center for Sports Medicine in July 2011. She will serve as the Athletic Trainer for Bradford and Newton High Schools.
Amanda received her Masters of Science degree in Exercise and Health Studies with a concentration in Exercise Science from Miami University in Oxford, OH in July 2011. Amanda also received her Bachelor of Science degree in Athletic Training and Exercise Physiology from Ohio Northern University in Ada, OH in May 2009.
Amanda is licensed by Ohio Athletic Training board and is certified by the National Athletic Trainers' Association. She is also certified from the ACSM as a Clinical Exericise Specialist. She is also an OHSAA certified general weight assessor and a certified Sportsmetrics instructor.
Joycelyn M. Kastl, LAT, ATC, Athletic Trainer, UVMC Center for Sports Medicine
Joycelyn Kastl has been an athletic trainer at UVMC Center for Sports Medicine since 1999. She provides athletic training services for St. Paris Graham High School, as well as assists in the clinic setting and with community events. In addition, she is also an Advanced Clinical Instructor for Urbana University interns in the field of athletic training.
Joycelyn received her Master of Arts degree in Physical Education from Western Michigan University in Kalamazoo, MI. She also received her Bachelor of Science degree in Sports Medicine from Mount Union College in Alliance, OH.
Joycelyn is licensed by the Ohio Athletic Trainers’ Board and is certified by the National Athletic Trainers’ Association. She is also an OHSAA-certified wrestling general weight assessor, certified CPR instructor, as well as an Advanced Clinical Instructor certified by the National Athletic Trainer’s Association. In addition, Joycelyn is a member of the Ohio Athletic Trainers’ Association Critical Incident Stress Management Team and a member of the Greater Dayton Athletic Trainer’s Association.
Matt Lundin, Ed.D, ATC, LAT, Athletic Trainer, UVMC Center for Sports Medicine
Matt joined the team of UVMC Center for Sports Medicine staff as an athletic trainer in the summer of 2012. Matt will be serving as athletic trainer for Covington High School, as well as other various community events.
Matt received his Master's degree in Physical Education with an option in Athletic Training from California State University in Chico, CA in 2003. He also received his Bachelor's degree from Rocky Mountain College in Billings, MT in 1997 in K-12 Physical Education and Exercise Science. Currently Matt is pursing his Doctorate in Education from Nova Southeastern University in Health Care Education.
Matt is also an OHSAA certified general weight assessor and is CPR and AED certified.
Zachariah Rahe, ATC, LAT, Athletic Trainer, UVMC Center for Sports Medicine
Zach joined the staff of UVMC Center for Sport Medicine as an athletic trainer in the summer of 2012. Zach will serve as athletic trainer for Troy Christian High School, and will assist in the clinical setting at UVMC's Outpatient Care Center South.
Zach received his Bachelor's degree from Wright State University in Psychology in 2005. He also received a secondary Bachelor's degree from Wright State University in Athletic Training in 2011.
Zach is also an OHSAA certified general weight assessor and is CPR and AED certified.
Tiffany R. Rhoades, LAT, ATC, CSCS, Athletic Trainer, UVMC Center for Sports Medicine
Tiffany Rhoades has been a part of the Upper Valley Medical Center team since 2005. She has been the assistant athletic trainer for Troy High School since 2005, as well as assisting in the clinic setting and with community athletic events. As a Certified Strength & Conditioning Specialist, Tiffany is one of the instructors for the ESP Jr. and ESP Programs.
Tiffany received her Bachelor of Science in Athletic Training from Urbana University in Urbana, OH in 2005. She is certified in CPR/ AED, as well as being a Certified CPR Instructor and an OHSAA-certified wrestling general weight assessor. She also has her National Athletic Trainer’s Association Certification and Ohio Athletic Training License.
Aaron Schlotterbeck, LAT, ATC, MBA, Athletic Trainer, UVMC Center for Sports Medicine
Aaron Schlotterbeck has been a part of the UVMC Center for Sports Medicine team since 2008. He serves as the athletic trainer for Tippecanoe High School, and has been working in the field of athletic training since 1997. He assists with the marketing and the development of new sports medicine-related programs for Upper Valley Medical Center.
Aaron received his Bachelor of Science degree in Sports Medicine from Urbana University in Urbana, OH in 1997. Aaron also received his Masters of Business Administration from Findlay University in 2009.
