Hyatt Center
450 N. Hyatt St., Tipp City
Monday through Friday: 7:30 am to 6:30 pm
Saturday and Sunday: Closed

Outpatient Care Center/North
280 N. Looney Rd., Piqua
Monday, Tuesday, and Thursday: 7:30 am to 4:00 pm
Wednesday and Friday: 8:00 AM to 4:00 PM
Saturday and Sunday: Closed

Upper Valley Medical Center
3130 N. County Rd. 25-A, Troy
Monday and Wednesday: 7:30 am to 7:00 pm
Tuesday, Thursday, and Friday: 7:30 am to 3:30 pm
Saturday: 7:30 am to 3:00 pm
Sunday: Closed
