When you or a family member is hospitalized at Upper Valley Medical Center (UVMC), your needs can go beyond the expert medical care provided by the hospital's health professionals. That's where the UVMC Social Services department comes in. Our caring, specially trained staff understands about the worries and stresses associated with times of illness. They are here to help you deal with personal, family or financial concerns that may arise during your hospital stay. 
 
While in the Hospital

Sometimes patients just need someone to talk with about the uncertainty and fears they are experiencing. They may feel anxious about their family's reaction to their illness and about child and family care while they are hospitalized. Many worry about the financial pressures associated with their illness. Those with long-term conditions may require ongoing rehabilitation or convalescence, which adds to their worries.
 
Our social workers are available to listen and offer support. They can help you sort out your concerns and work with you on finding practical solutions to your problems. 
 
Planning Ahead

Proper follow-up care after hospitalization is important for all patients. For some, that may mean care in the home, and for others it may include a stay in a nursing home. Whatever your needs are, the UVMC Social Services staff will work with you and your family to assure that you will receive the ongoing care you require following discharge from the hospital.
