To schedule a test, Pre-register or obtain a Price Estimate
Please call (937) 499-7364 or (855) 887-7364. Once you are scheduled for your services, if a pre-certification is needed, we will begin that process.
Patients who come to UVMC for medical imaging should report to the Outpatient Services Registration desk in the main lobby. Scheduled procedures including CT, MRI, Nuclear Medicine, Ultrasound, Mammography, and Fluoroscopy should arrive as directed by Central Scheduling. Walk-in appointments for other imaging procedures or x-rays are welcome. Hours of operation for medical imaging are 6:00 am to 9:30 pm Monday-Friday and 7:00 am to 3:30 pm Saturday.
Patients coming for EKGs and laboratory tests should also report to the Outpatient Services Registration desk. These are mostly walk-in appointments and are performed 6:30 am to 6:00 pm Monday-Friday and 7:00 am to 12:00pm on Saturday.
Patients who come to UVMC for surgery or preadmission testing (PAT) should report to the Surgery Welcome Center on the second floor. Registrar hours are 5:30 am to 1:30 pm. After 9:30 pm on weekdays or 3:30 pm on weekends, patients are asked to enter through the Emergency Department and check in at the ED front desk.
Patients are registered in order of arrival. The registrar will collect or update patient demographic information, have the patient sign required forms, scan insurance cards and other documents, and collect co-pays. Hospital escorts will help any patient requesting a wheelchair or assistance.
Registration Documents to Bring to the Hospital
Outpatients or patients checking in are advised to bring the following documents with them to make the registration process faster and easier:
- Physician’s order for test/admission if your physician provided one
- Insurance card
- ID/Driver’s License
For the convenience of patients, their families, and visitors, Medical Center employees are asked to park only in specially designated areas. Parking designations include:
- Medical office staffs in the Physicians Office Building
- Maternity patients, families, visitors
- Medical Staff, employees
We maintain a hospital directory listing the name, room and extension, general condition and, if you wish, your religious affiliation. Unless you choose to have your information excluded from this directory, the information excluding your religious affiliation will be disclosed to anyone who requests it by asking for you by name. This information, including your religious affiliation, may also be provided to members of the clergy. You have the right during registration to have your information excluded from this directory and to restrict what information is provided and/or to whom.
If you choose to be excluded from the hospital directory, you will receive no calls, mail, or flower delivery. The hospital will tell callers—including family members, friends, and clergy that you have not given your direct hospital phone line to—“I’m sorry, this person is not in our hospital directory.”
For reasons of privacy and confidentiality, no information will be revealed to anyone about a patient who chooses not to be included in the hospital directory.
