Premier Health is committed to providing pricing information to help you make informed health care decisions. Premier Health provides various options to provide you estimated pricing information for hundreds of items and services.

Patient Price Estimator

An estimate for certain items and services may be generated electronically using our self-service price estimator tool or by calling and speaking with a specialized patient financial counselor. You can access these tools through the options listed below.

  • If you would like to speak with a patient financial counselor to receive a patient specific estimate, please contact our price line at (937) 499-7364 option 5 or toll-free at (855) 887-7364 Monday through Friday 8:30 a.m. to 4:00 p.m.
  • If you have a MyChart account with Premier Health, please login through your MyChart account for a patient specific estimate.
  • If you do not have a MyChart account with Premier Health, please use our guest access to access a patient specific estimate.

To receive an estimate, you will need detailed information about your upcoming item(s) and service(s). Please be prepared to provide any insurance information, location of planned service(s) and names or CPT/HCPCS codes for planned services.

The charges listed are an estimate only and specific to the hospital charges. They are not intended to be a quote and do not include any physician fees billed by your surgeon, anesthesiologist, emergency specialist, pathologist, radiologist, or other physicians who may help with your care. Your actual charges could be more or less than these estimates depending on the specific factors of your case such as specific tests ordered by your doctor, previous test results, medical history, other services unique to your specific health care needs, etc.

Usual and Customary Charges Premier Health

Per Ohio Administrative Code, the below files meet the requirements outlined in section 3727.12 regarding usual and customary charges.

Fidelity Health Care Pricing

Estimated pricing for your home health, home infusion or medical equipment is provided to you by the Fidelity staff. The pricing is an estimate based on information provided to us by your insurance company. Exact pricing is not always available to us until your claim is processed by your insurance company.

Be prepared to make a co-payment or down payment on your coinsurance when you start receiving services or equipment from us. You may be asked to pay in full for non-covered services before care or equipment is provided.

In addition to working with insurance carriers, all of our services are available on an Independent Care Services basis as well. We accept Visa, MasterCard, and Discover credit cards, as well as checks and cash.
